Confidentiality Agreement
CONFIDENTIALITY AGREEMENT
This agreement is between you and Airdrie & District Hospice Society. Please carefully read all the terms and conditions set out below. Signing this agreement indicates your acceptance of the terms and conditions of this agreement and that you intend to be legally bound by them. If you do not agree with the terms and conditions, you are responsible for advising the Program Director or Board of Directors.
WHEREAS: 1. Airdrie and District Hospice Society is bound as defined as a non-profit organization as defined in the Personal Information Protection Act, AB (PIPA) and the Personal Information Protection and Electronic Documents Act, CANADA (PIPEDA).
I AGREE THAT:
1. All Personal and Business information that I collect, use, retain and/or disclose in my role as an Affiliate of Airdrie and District Hospice Society is private and confidential.
2. It is my responsibility as an Affiliate of Airdrie and District Hospice Society to know and follow relevant privacy and information security policies in effect at Airdrie and District Hospice Society.
3. I will take all reasonable steps to act in accordance with applicable Airdrie and District Hospice Society policies, bylaws, PIPA, PIPEDA, and any other privacy legislation in effect and to keep private and confidential and prevent the unauthorized collection, use and/or disclosure of all Resident, Personal and/or Business information that I come in contact with in my role as an Affiliate of Airdrie and District Hospice Society. Such steps include, without limitation, taking responsible security precautions against such risks as unauthorized access, collection, use, disclosure, alteration, or disposal.
4. If I knowingly collect, use and/or disclose Client Information, Personal Information and/or Business Information in my role as an Affiliate of Airdrie and District Hospice Society in contravention of Airdrie and District Hospice Society policies, bylaws, PIPA, PIPEDA, and any other privacy legislation in effect, I may be subject to disciplinary action, termination and/or guilty of an offence under PIPA, PIPEDA, and any other privacy legislation in effect.
5. I am responsible to keep confidential all Client and Resident Information, Business Information and Personal Information for as long as required by PIPA, PIPEDA, or other relevant privacy legislation in effect.
6. I agree to notify Airdrie and District Hospice Society as soon as reasonably possible if I am aware of a breach of this agreement.
