Please acknowledge and agree to the following before completing the application:

• I am a high school student between the ages of 16 and 17.
• I acknowledge that my parent or guardian is aware of my intention to volunteer at Endeavor Health and the commitment involved.
• I understand that completing this application does not guarantee a volunteer assignment for me.
• I acknowledge the volunteer requirement is a minimum scheduled commitment of 4 hours per week and 100 hours for the year (a minimum of 2 hours per week for Swedish Hospital during the school year)
• Applications will be considered on a rolling basis. We hold applications for one year and consider applicants as our needs change.
• I understand as a condition of initial and continued participation in the volunteer program I agree to complete:

• 1. Online volunteer orientation
  2. All health requirements as determined by Endeavor Health Employee Health Department

• If accepted I will offer my services without monetary compensation.
• If accepted I agree to comply with all the hospital’s rules and regulations and those specific to the volunteer services department
