Privacy Statement: Personal information on this form is collected under the authority of s.11(1) of the Municipal Act and in accordance with s. 28(2) of the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA), and will be used to qualify you as a City of Kitchener Volunteer. This information will be shared with staff who require it for their work as part of administering and supporting of volunteers. All information you provide will be stored on a Third Party server located in Canada.
