Court-Appointed / CSR Program Application

When you start and complete your application, you are agreeing to our Community Engagement Department and PVAS Policies. Read our Volunteer service agreement here.
All volunteers must not only adhere to our policies but also review and understand our Court-Appointed / CSR Program Policies

If you have your Agency Contact Sheet & CSR Timesheet or any Court Documents for your CSR please upload them to your profile or Scan and Email them to volunteer@pvastx.org type "CSR Paperwork" in the Subject line.

Please allow 24-72 hours to review your application. Once a decision has been made, we will reach out via email, call, or text. *Be sure to check your Spam/Junk Folder*

To begin your application, Fill out the fields in the "I am new to MyImpactPage" box, click on "I Agree to adhere to PVAS policies", and click the "Save & Continue" button.

Si tienes tu hoja de Agency Contact Sheet y la hoja de CSR Timesheet, o cualquier documento judicial, por favor, cárgalos en tu perfil o escanéalos y envíalos por correo electrónico a volunteer@pvastx.org con "Documentos de CSR" en la línea de asunto.
