GOSH Voice Member- Aged 12-15

 

Welcome!

We use an online system called MyImpact to manage GOSH Voice applications and activities and to keep your information secure.

If you are aged between 12-15, you're in the right place!

If you are aged 16+: Please use this form

If you are aged under 12 years old: Please use this form

 

How to sign up: 

  1. Scroll down to “I am new to MyImpactPage.com” at the bottom of this page. 
  2. Create a username.
  3. Enter your email address.
  4. Click 'Save and Continue'.
  5. You will be taken to the GOSH Voice (16+) application form.

Once you have signed up, we will email your parent/carer to confirm they are happy for you to take part. We will need them to complete and return a consent form, before we can approve you as a member. 

 

 

Quick reminder!- Who can become a member of GOSH Voice?

  • Patients. If you are currently receiving care or treatment at GOSH.
  • Siblings. If your brother or sister is a current patient at GOSH.
  • Parents, carers and extended family. If your child/family member is a current patient at GOSH.
