Organization Policies
By agreeing to the organization's policies, I hereby state that I have chosen to participate in this City of Scottsdale Special Event, and that in exchange for the opportunity to participate in the Event I am entering into this Indemnity and Release Agreement (the Agreement). I am aware that participation in the Event may involve certain risks that may cause injury or serious injury. If the participant is a minor, then I state that I have the authority to enter into this Agreement on behalf of such minor and that the statements contained herein pertain both to such minor and to myself.
By entering into this Agreement I release the City of Scottsdale, its elected officials, departments, agencies, officers, agents, volunteers or employees, and all other persons acting through or on behalf of the City of Scottsdale, from any claim that I and/or the minor for whom I am signing may have arising or resulting from participating in the competition for injury, damage, loss, or expense, including claims arising from the negligence or gross negligence of any person released herein, and agree, to the fullest extent permitted by law, to indemnify and hold harmless any person released herein for any such claim made by me or by any other person or entity, including by any minor on whose behalf I may be signing this Agreement.
I hereby authorize the City of Scottsdale staff to obtain any needed medical assistance for myself and any minor on whose behalf I may be signing this Agreement in case of an emergency, illness, or accident. I understand that any resulting expenses or charges are my responsibility and I will pay them immediately, either directly or through personal insurance.
I am aware that photographic or other representations of me/such minor may be created during the Event and I release any claim by me/such minor against the City of Scottsdale, its elected officials, departments, agencies, officers, agents, volunteers, or employees, for any use of such representation in historical documentation, promotional, or any other activities or uses.
I understand that if I am 18 years of age or older, I must provide an accurate full legal name and the date of birth, which the City may use to perform a background check including a sex offender registry search. I understand that the City does not allow individuals to volunteer who have been convicted of a violent crime, a sex offense, or a crime of dishonesty, and I hereby swear and affirm that I have not.
