Foster Agreement & Liability Waiver
By checking the "I AGREE" box below, I acknowledge and agree to the following:
- I confirm that all information provided on this application is true and accurate.
- I agree to serve as a responsible and cooperative member of the Tallahassee Animal Services (TAS) Foster Program and will follow all TAS policies and procedures as explained during the Foster Parent Orientation and outlined in the Foster Parent Handbook.
- If my foster animal requires medical attention, I will notify TAS before seeking veterinary care. I understand that any unauthorized veterinary expenses will not be reimbursed.
- I acknowledge that my participation in the foster program is voluntary, and I will not receive compensation for any time spent as a foster volunteer.
- I give my consent to TAS to use my name, voice, photograph, or images of any animal in my care for media, advertising, programming, or operational activities. I understand that I will not receive compensation for this usage.
- I agree to hold harmless Tallahassee Animal Services, the City of Tallahassee, Leon County, and their representatives, including agents, employees, directors, and insurance carriers, from any actions, claims, damages, or judgments related to my participation in the TAS Foster Program, including but not limited to personal injury.
