Volunteer Requirements, Disclaimers, and Health Screening

VOLUNTEER STATEMENT & LIABILITY/PHOTOGRAPHIC RELEASE
I understand that The Salvation Army is a religious and charitable organization that requires
the assistance of volunteers in the conduct of its various spiritual and social service programs.
It is my desire to further the work of The Salvation Army by performing services as a volunteer.
I understand to perform said services as a volunteer without compensation, and that in performing said services, I acknowledge that I am not acting as an employee of The Salvation Army.
I understand that as a volunteer, should any accident occur, The Salvation Army will not be responsible for any injury to myself or loss/damage of personal property.
I hereby give my permission to be photographed or video recorded without compensation by a representative of The Salvation Army for the sole purpose of promotion of the services available at The Salvation Army. I also understand that I have the option at any time not to be photographed and not to be present if I choose.

[NEW POLICY]: I acknowledge that Salvation Army does not accept or verify hours for court-ordered community service. I certify that I am seeking a traditional volunteer role and am not attempting to fulfill a legal or court-mandated requirement.

Wondering if you should register to volunteer? we are asking you the following questions:

Have you been exposed to someone who has been diagnosed with COVID-19 in the last 5 days?

Do you have symptoms consistent with COVID-19? Symptoms can include a cough or shortness of breath or at least two of the following: Fever, Chills, Repeated shaking with chills, Muscle pain, Headache, Sore throat, New loss of taste or smell.
- Are you seeking to fulfill a court-ordered community service requirement? (Please note: This location does not accept or verify hours for court-mandated service.)
If you answered "yes" to any of these questions, we ask you not to sign up for an activity at this time. We are doing all we can to align with best practices to keep our community healthy.
