Welcome to the Patient & Family Partner Program at LHSC and Children's Hospital!
We are excited to invite you to join a community dedicated to enhancing patient care by incorporating the invaluable insights of patients, families, and caregivers. Our program bridges the gap between hospital staff, physicians, and leaders, ensuring that the patient perspective is integral to the development and improvement of our services.
Who is a Patient or Family Partner?
A Patient or Family Partner is someone who has recently (within the last three years) experienced being a patient or the caregiver of a patient. By collaborating with our hospital team, you will provide direct input into policies, programs, and practices that shape patient care and services. This collaborative effort, known as an Opportunity for Engagement, aims to create a healthcare environment that truly reflects the needs and preferences of those we serve.
Our Selection Process:
To ensure a meaningful partnership, we follow a thorough screening process that includes:
- An introduction phone call/virtual meeting to understand your experiences and perspectives.
- A police record check to maintain a safe and secure environment.
- An internal review for any unresolved Patient Relations files.
- Participation in an LHSC Patient and Family Partner Orientation session to familiarize you with our program and expectations.
