Privacy Act Declaration: I acknowledge that Dress for Success Auckland needs to collect personal information about me for the purpose of assessing suitability, and engaging with me as a Dress for Success Auckland volunteer. Such information will include this form and the Police check request form or the current clearance you will provide to us (current within the past 6 months).
This information will be used for this purpose only and can be viewed by me at any time for review or alteration. I consent to this information being provided and used by the Ministry of Social Development or other agency or organization which provides funding to Dress for Success for the purpose of assessing my suitability as a volunteer.
I acknowledge that signing this form confirms my consent to the use of my information for the above purpose.
Confidentiality: I undertake to keep all information that I acquire through Dress for Success Auckland including the identity of clients, sponsors, staff and other volunteers, strictly confidential.
Police check: I have completed and submit with this application a Police Check request which will be submitted by Dress for Success Auckland to the Ministry of Justice. I consent to the disclosure by the Ministry of Justice of any information it may have pursuant to the attached application to Dress for Success Incorporated. I understand that any record of criminal convictions I might have will be automatically concealed if I meet the eligibility criteria stipulated in section 7 of the Criminal Records (Clean Slate) Act 2004 or
If I have a cleared Police Check from the Ministry of Justice that is no more than 6 months old, I will provide that to Dress for Success Auckland to support my application.
I confirm that I do not have any convictions for fraud under the Social Security Act 1964 or the Crimes Act 1961.
