Agreement Of Confidentiality

 I __________________________________ of the Town of                                                  in the Province of Alberta agree to the following terms and conditions governing my work as a volunteer for the Okotoks Food Bank Association (OFBA).

1. All information given by any user for the OFBA whether given orally, in writing or in any other form is to be kept strictly confidential except, for being exchanged with the staff or volunteers working in the OFBA in the fulfillment of duties as a volunteer.
2. I, knowing the above stated policy, agree that should I deliberately or accidentally breech

term 1, I accept full responsibility for both the breech and any and all consequences, and that the OFBA is hereby exempt from any and all losses that may result from any act or omission

on my part.

3. I understand that failure to observe the above mentioned terms may result in my dismissal from the OFBA.
4. I affirm that I understand and freely consent to the above mentioned terms and conditions.

Code Of Conduct Agreement

[   ] Our words behaviours and decisions impact our food bank community so I will treat clients, volunteers, staff members and directors with the dignity and respect we would like to receive.

[   ] I will treat all those who access services of the OFBA with the utmost dignity and respect regardless of race, national or ethnic origin, citizenship, colour, religion, sex, sexual orientation, income source, age or mental or physical ability.

[   ] I will implement best practices in the proper and safe storage and handling of food.

[   ] I will not accept payment of any kind for donated food or consumer products.

[   ] I agree to participate in training on the occasion that it may be required.
