General Data Protection Regulations and Data Protection
Guy’s and St Thomas’ NHS Foundation Trust need to collect personal information about you in order to process your application form. It will also form the basis of a confidential personal record in electronic and paper format. The data will be retained for administrative and statistical reporting purposes.
Following completion of your volunteer role it will be held for five years and will then be destroyed under secure conditions if you are accepted as a volunteer. If you are not short listed for a role or do not become a volunteer it will be destroyed within three months of receipt.
In accordance with the General Data Protection Regulation and Data Protection Act 2018, the information provided on this form will only be disclosed to those who have a legitimate reason to see it.
The lawfulness of processing this information under the General Data Protection Regulation and the Data Protection Act 2018 is that the processing is necessary for the performance of a contract, 6(1)(b).
The Trust would like to make you aware that you have the following rights regarding the data we are required to collect about you:
- make a Subject Access Request
- rectification of any incorrect information we hold about you
- restriction of further processing data
- right to be forgotten where information has been provided with consent

We will keep in touch with you while you are in the application process and during your time volunteering at Guy's & St Thomas’ NHS Foundation Trust and this will be primarily through e-mail and text messaging.
