I understand that to be eligible to volunteer with MOSAIC I must be living in Canada. *

I understand that MOSAIC serves vulnerable populations and many volunteer roles require Criminal Record Check/Criminal Background Checks to participate.

I understand that MOSAIC collects certain personal information to assist with matching me to an appropriate role. I understand that my personal information will not be used or communicated outside of this program without my direct permission.

I understand that MOSAIC will keep my personal information private in accordance with local, provincial and federal legislation including but not limited to the Freedom of Information and Protection of Privacy Act (FOIPPA), Personal Information Protection and Electronic Documents Act (PIPEDA )

*International Practicum Placement Students are hosted by MOSAIC from time to time.
