GOSH Voice Member- 16+

We use an online system called MyImpact to manage GOSH Voice applications and activities and to keep your information secure.

If you are aged 16 or over: you're in the right place!

If you are aged 12-15 years old: Please use this form

If you are under 12 years old: Please use this form

 


How to sign up: 


  1. Scroll down to “I am new to MyImpactPage.com” at the bottom of this page. 
  2. Create a username.
  3. Enter your email address.
  4. Click 'Save and Continue'.
  5. You will be taken to the GOSH Voice (16+) application form.

 


Quick reminder!- Who can become a member of GOSH Voice?


  • Patients. If you are currently receiving care or treatment at GOSH.
  • Siblings. If your brother or sister is a current patient at GOSH.
  • Parentscarers and extended family. If your child/family member is a current patient at GOSH 
  • Bereaved parents and carers. If your child previously received care at GOSH.
  • Alumni. If you are previous patientsibling or parent/carer within the past 5 years
