Volunteer Policies and Agreements
This waiver releases Street Dog Hero (SDH), a non-profit corporation organized and existing under the laws of the State of Oregon and each of its directors, officers, employees and volunteers. The Volunteer desires to provide volunteer services for SDH and engage in activities related to serving as a volunteer. Volunteer understands that the scope of Volunteer’s relationship with SDH is limited to a volunteer position and that no compensation is expected in return for services provided by Volunteer; SDH will not provide any benefits traditionally associated with employment to Volunteer; and that Volunteer is responsible for his/her own insurance coverage in the event of personal injury or illness as a result of Volunteer’s services to SDH. In consideration for being permitted by SDH to participate as a volunteer in various activities, I hereby waive, release, and discharge any and all claims and damages for personal injury, death, or property damage which I may sustain or may which hereafter accrue as a result of my participation as a volunteer and resulting from negligence or misconduct of Street Dog Hero.
1.Waiver and Release: I, the Volunteer, release and forever discharge and hold harmless SDH and its successors and assigns from any and all liability, claims and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from the services I provide to SDH. I understand and acknowledge that this Release discharges SDH from any liability or claim that I may have against SDH with respect to bodily injury, personal injury, illness, death or property damage that may result from the services I provide to SDH or occurring while I am providing volunteer services.
2.Insurance: Further I understand that SDH does not assume any responsibility for or obligation to provide me with financial or other assistance, including but not limited to medical, health or disability benefits or insurance of any nature in the event of my injury, illness, death or damage to my property. I expressly waive any such claim for compensation or liability on the part of SDH beyond what may be offered freely by SDH in the event of such injury or medical expenses incurred by me. I understand that SDH does not maintain an insurance policy covering any circumstances arising from my participation in any volunteer activities.
3.Medical Treatment: I hereby Release and forever discharge SDH from any claim whatsoever which arises or may here after arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my tenure as a volunteer with SDH.
4.Assumption of Risk: I understand that the service I provide to SDH may include activities that may be hazardous to me including, but not limited to, bites, scratches, jumped on, dragged, knocked over, slips, trips, falls, ringworm, parasites, zoonotic illness, lifting injuries, loss of personal property, damage to apparel or personal property or exposure to cleaners, involving inherently dangerous activities. As a volunteer, I hereby expressly assume the risk of injury or harm from these activities and Release SDH from all liability for injury, illness, death or property damage resulting from the services I provide as a volunteer or occurring while I am providing volunteer services.
5.Photographic Release: I grant and convey to SDH all right, title and interests in any and all photographs, images, video or audio recordings of me or my children or my likeness or voice made by SDH in connection with my providing volunteer services to SDH.
6. Other: As a volunteer, I expressly agree that this Release is intended to be as broad and inclusive as permitted by the laws of the State of Oregon and that this Release shall be governed by and interpreted in accordance with the laws of the State of Oregon. I agree that in the event that any clause or provision of this Release is deemed invalid, the enforceability of the remaining provisions of the Release shall not be affected.
7. Hold Harmless Agreement By signing below, I hereby accept a position as a Volunteer for the SDH upon the following terms, conditions and understandings: My services to SDH are provided strictly in a voluntary capacity as a Volunteer, and without any express or implied promise of salary, compensation or other payment of any kind whatsoever. My services are furnished without any employment-type benefits, including employment insurance programs, unemployment compensation, and workers compensation accrual in any form, vacations or sick time. I will familiarize myself and comply with SDH policies and procedures applicable to Volunteers. In particular, I fully understand that SDH expects high standards of moral and ethical treatment of the animals under its care. I will adhere strictly to these standards in my capacity as a Volunteer. I understand that I am a representative of SDH, and I will present myself in a professional manner including good hygiene and proper clothing attire. I will not make decisions on behalf of the organization or the animals in the organization or provide opportunities that are not approved by the Coordinators or Board of Directors. I understand that the handling of animals and other Volunteer activities including but not limited to parades, adoption events, general events, fundraising events, sterilization clinics and fostering on behalf of SDH may place me in a hazardous situation and could result in injury to me or my personal property including death. On behalf of myself, and my heirs, personal representatives and assigns, I hereby release, discharge, indemnify and hold harmless SDH and it’s directors, officers and volunteers from any and all claims, causes of action and demands of any nature, whether known or unknown, arising out of or in connection with my Volunteer activities on behalf of SDH. By electronically submitting my name below, I express my understanding and intent to enter into this Release and Waiver of Liability willingly and voluntarily.
