Volunteer Release and Waiver of Liability

This Release and Waiver of Liability is made in favor of Above and Beyond Food Pantry (AB Food), a nonprofit organization operating under the laws of the State of Illinois, as well as its directors, officers, employees, and agents. By signing this form, the Volunteer acknowledges their desire to provide services to AB Food and participate in volunteer activities. The Volunteer understands that their role is strictly that of a volunteer, with no expectation of compensation or employment benefits. Furthermore, AB Food will not provide any benefits typically associated with employment, and the Volunteer is responsible for their own insurance coverage in the event of personal injury or illness arising from their volunteer work with AB Food

1. Waiver and Release: I hereby release, discharge, and hold harmless Above and Beyond Food Pantry, along with its successors and assigns, from any and all liability, claims, or demands of any kind, whether in law or in equity, that may arise from the services I provide as a volunteer for AB Food. This includes any claims related to bodily injury, personal injury, illness, death, or property damage that may occur as a result of my volunteer activities or while I am performing volunteer services. I understand that this waiver prevents me from holding AB Food liable for such occurrences.

 

2. Insurance: I acknowledge that AB Food does not provide me with any form of financial assistance or insurance coverage, including medical, health, or disability benefits. I fully waive any claim for compensation or liability from AB Food regarding these benefits.

 

3. Medical Treatment: I release and forever discharge AB Food from any claims arising from any first-aid or medical treatment I receive in connection with an emergency that occurs during my time as a volunteer.

 

4. Assumption of Risk: I understand that my volunteer work may involve activities that carry inherent risks, such as driving, lifting, pushing, pulling, and working with cleaning chemicals, among others. I voluntarily assume all risks of injury or harm that may arise from these activities and agree to release AB Food from any liability related to such risks.

 

5. Photographic Release: I grant AB Food all rights to any photographs, images, videos, or audio recordings of me or my likeness that are taken in connection with my volunteer services. AB Food may use these materials for any purpose without requiring further permission from me

 

6. Other: I agree that this Release is intended to be as broad and inclusive as allowed by the laws of the State of Illinois, and that it will be governed and interpreted accordingly. I agree that in the event that any clause or provision of this Release is deemed invalid, the enforceability of the remaining provisions of this Release shall not be affected.

 

Volunteer Expectations

  1. Wear sensible, non-offensive, appropriate clothing and footwear for the task(s) at hand. Closed toed shoes are highly recommended for all activities. AB Food aprons cannot be worn outside the pantry. AB Food aprons must be returned after each shift.
  2. Wash hands before beginning your shift, after eating, and after using the restroom. Gloves should always be worn while handling food at the pantry. 
  3. Alcohol and other drugs are prohibited in the workplace. All smoking activities must be done outside and 15'+ away from all exterior doors of the pantry.
  4. Report any injury immediately to AB Food staff on site.
  5. No food, drink (exception to closed bottles of water), or cell phone use on the pantry floor. If you must use your cell phone please relocate to an office or the volunteer area.
  6. All volunteers will treat other volunteers, clients and staff in a respectful manner.

Volunteers cannot take food from the food pantry during the duration of their shift. There is a specific time set up for volunteers to shop. You may not take food from the back store room or the fridges and freezers without asking permission from staff.
