Inspiritus Volunteer Policies
Confidentiality: I acknowledge that all Inspiritus client-related or donor-related information is to be kept confidential by the volunteer. Client information, including but not limited to client-related records and the information contained in them, may be released only if authorized by law and in accordance with Inspiritus policies and procedures. I acknowledges that policies and procedures with regard to client or donor confidentiality have been explained to them and that I understands those policies and procedures.
Media: I hereby consent, authorize and grant permission to the employees or representatives of Inspiritus for the collection and use of my personal images by photography and audio/video recording and do further consent to publication, circulation, or dissemination of said media for any purpose Inspiritus deems appropriate.
Inspiritus Publications: I hereby give my permission for Inspiritus to send me newsletters and other agency publications via email or postal mail.
Reporting: I Agree to submit a monthly record of my service. I also agree that all time, mileage and donation information indicated on reporting forms that I submit to Inspiritus is true and complete, unless I notify Inspiritus in writing of errors or omissions within 5 business days of original submission.
Electronic Submission Forms: I Hereby agree that my typed signature on forms electronically submitted to Inspiritus represents my written signature.
Vehicle Liability: It is the policy of Inspiritus to ensure that all drivers practice safe driving, provide safe transportation, and ensure proper reporting of all vehicle accidents when operating vehicles on behalf of Inspiritus. You agree to ensure that all vehicles used for the transport of accompanied persons are in adequate condition and appropriate to their mental and physical needs.
Background Check: It is the policy of Inspiritus to conduct background checks on all new employees, consultants, independent contractors, volunteers and student interns who provide direct services to or are alone with persons determined to be a vulnerable population or work with sensitive or confidential information.
I understand that to work directly with clients of Inspiritus or have access to sensitive information, I must consent to a background check to be conducted by Inspiritus, including but not limited to motor vehicle record check( if work includes driving Inspiritus clients), child abuse registry, sex offenders and other national and state criminal registry. Please advised that the information obtained in this release form and through the background check will remain confidential and will be viewed only by selected Inspiritus staff. Inspiritus reserves the right to re-check the criminal history of any volunteer when information or circumstances indicate the need.
Withdrawal of Consent: I understand I can withdraw my consent to this release agreement at anytime in writing to Inspiritus at the address listed below.
