Volunteer Service Agreement

I certify that all information on my volunteer application to The Seattle Public Library is accurate. In expressing interest in a volunteer opportunity, I give permission for, and understand that with proper notification to me, The Seattle Public Library may conduct a criminal history check on me. This background check is in compliance with the Child/Adult Abuse Information Act (RCW 43.43.830 through 43.43.840).

Maintain Confidentiality: I understand that it is the policy of The Seattle Public Library to protect the privacy of library patrons. I agree to hold all information about patrons in complete confidence and to access this information only in the course of performing my volunteer service. I also understand that a breach of confidentiality is grounds for dismissal from the Volunteer Services Program.

Volunteer Service Hours: I agree to report my hours through the MyImpactPage.com portal each time that I volunteer. I will only report hours that I have actually completed.
