Liability, Photo and Confidentiality Release
Liability Release. I understand that volunteering with the Family Support Center of Ogden may involve certain risks. I voluntarily assume these risks and agree to release and hold harmless the Family Support Center of Ogden, its directors, employees, volunteers, and representatives from liability for injury, illness, loss, or damage arising from my volunteer service, except in cases of gross negligence or willful misconduct. I agree to follow all agency policies, procedures, safety guidelines, and instructions.
Photo and Media Release. I grant the Family Support Center of Ogden permission to photograph, video record, or otherwise capture my image and likeness during volunteer activities for use in organizational publications, social media, websites, fundraising, marketing, and educational materials. I understand that I will not receive compensation and may withdraw consent for future use by providing written notice; however, this will not apply to materials already created or published.
Confidentiality Agreement. I understand that I may have access to confidential information regarding children, families, clients, employees, donors, or agency operations. I agree to keep this information private and confidential during and after my volunteer service and will not share, discuss, photograph, or disclose any identifying or confidential information unless authorized by the Family Support Center of Ogden or required by law.
